What information do we collect?
Renew Physical Therapy Specialists collects your medical and contact information such as email, phone numbers, mailing address, health insurance information and any past medical history through our intake form that we will send you via email or you can fill out in office after making an appointment with us. We store that information on a HIPAA compliant EMR system.
Why do we collect that information?
We collect your information specifically to provide you with the best possible Physical Therapy care and in order to bill your health insurance company after your visits. We use your contact information to schedule, cancel, and reschedule your appointments and contact you about any outstanding copays, deductibles, or coinsurances you may owe after your visits have been processed through your health insurance.
We do not share your information with any third parties. We never use your information for marketing or promotional purposes. SMS opt-in or phone numbers for the purpose of SMS are not being shared with any third party and affiliate company for marketing purposes

Renew Physical Therapy Specialists uses text messaging conversationally specifically for scheduling, canceling, rescheduling, billing reminders, and sending appointment reminders. You can OPT in by clicking the corresponding box on our intake paperwork after scheduling an appointment.
The information (phone numbers) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.
If you have consented to receive text messages from Renew Physical Therapy Specialists, you may receive messages related to the following: Appointment Scheduling, appointment reminders, billing reminders
Example: This is a reminder of your Physical Therapy appointment scheduled for 10am on Tuesday. Reply STOP to opt out of text messaging anytime.
Message frequency may vary. You may receive up to (4) SMS messages per week regarding your account notifications
